For routine life changes (such as a new home address or job), your local Friend of the Court typically provides forms to correctly report the necessary information. Often there also are user-friendly forms to report more contentious matters as well, such as non-payment of medical expenses and missed parenting time.

Many other counties offer similar resources for their local forms online and, prior to COVID, pre-printed forms were often available for pick up or purchase at the court clerk’s office. If your matter is outside Livingston County, it is best to consult your county court’s website or call the office of the clerk for your court.
